My 12 yr old cat has one eye that is dilated what causes this?

A dilated eye on only one side is often related to an ear problem on that side. It could be a problem with its inner ear, sometimes caused by wax build-up deep in the ear. It could also be a sign of infection.
With older cats it can be a symptom, sometimes the only symptom, of a mild stroke.
I work at a vet hospital, and I can recall several cats who were brought in because one eye was dilated. One had an ear infection from a grass awn deep in its ear and we never did determine the cause with the other three or four. The eye returned to normal in two cases and stayed dilated in the others. None of them ever developed worse symptoms or had further strokes.
That being said, you should have your vet look at the cat if the eye doesn't return to normal in a few days.
