Answers: It's a Hemicyclaspis. They've be extinct for awhile now so I doubt you could find one at Petsmart :)

EDIT - No, certainly I'm right. The class is Agnatha but the genius is Hemicyclaspis. The fish is a Hemicyclaspis, a type of Agnatha.
